Policies
Late Policy
We ask that you arrive 15 minutes prior to your scheduled appointment so that we can have you checked in and ready to see the doctor when your appointment time starts. If you arrive after your appointment time, you will be asked to reschedule as a courtesy to patients who have scheduled appointments after you.
Cancellation Policy
We ask that you to call at least 24 hours prior to a scheduled appointment if you need to cancel. Same day cancellations will be counted as a no-show. This will help us to meet the needs of all patients.
No Show Policy
After 3 missed appointments (no-shows) you will be notified by certified mail that you will need to find alternative medical care within 30 days.
Refill Policy
Regular medical visits are required for refills of medications. Based on your condition, the frequency of visits may vary. Please call for a refill several days in advance of running out of your medication. It may take up to 48 hours for your refill request to be processed. If you are overdue for a visit or bloodwork, the doctor may require you to schedule an appointment before a full refill can be prescribed. It is the patient's responsibility to schedule an appointment before running out of medication.
Paperwork Policy
You will be asked to schedule an appointment to have medical forms or documents completed. Often, specific exam findings and questions need to be discussed for your doctor to complete these forms accurately.
Insurance Coverage Policy
We participate with most insurance plans. Please bring an up to date insurance card and ID to each visit. Knowing your insurance benefits is your responsibility. Please contact your insurance company for any questions you may have regarding your coverage.
Copays Policy
All copays must be paid at time of service. A bill will be sent out for any deductibles after we bill your insurance.
